Pathways to Life is searching for a Crisis Stabilization QMHP-A.  The Crisis Stabilization Qualified Mental Health Professional position is responsible for providing crisis treatment, individual and family counseling and interventions; life, parenting, and communication skills; case management activities and coordination with other services; and emergency response to adult in at risk situations.  The Qualified Mental Health Professional is supervised by, and reports to the Lead Mental Health Professional.

DUTIES AND RESPONSIBILITIES:

  • Provide behavioral support services to clients experiencing a crisis jeopardizing their current placement.
  • Initiate and facilitate de-escalation techniques and strategies to assist the client in maintaining self-control. 
  • Use good interpersonal communication skills to gain client cooperation and resolve conflicts.
  • Interact with all clients in accordance with the individual service plan and the therapeutic process.
  • Assess, evaluate and monitor the safety of the client and others at all times.
  • Exercise appropriate intervention in accordance with the least restrictive alternative to protect client from harming self or others.
  • Coordinate closely with other staff and community partners to ensure the appropriateness of treatment modalities and services being provided. 
  • Be available for 24 Hour Emergency Crisis Response to assigned participant
  • Serve as clinical liaison to referral sources and parents regarding progress and needs; to include attendance of FAPT meetings and court hearings as necessary. 
  • Develop and implement plans for discharge including mobilization of resources for ongoing maintenance, rehabilitation & recovery. 
  • Develop assessments, individualized service plans, progress notes, and discharge summaries.
  • Maintain high quality documentation of all case work, completed in a timely manner and consistent with outlined program policies and licensure requirements. 
  • Administer first aid to client in response to minor medical problems 
  • Notify Supervisor of any significant client medical complaint(s), unusual events, accidents, medical problems, property damage or maintenance concern, etc.
  • Provide support and linkage to community resources for participants
  • Provide transportation for participants to and from appointments toward the completion of the treatment goals and objectives.
  • Complete and submit all clinical documentation by specified deadlines and attend all company and state required staff meetings and supervisions.

Only direct face-to-face contacts and services to the member are reimbursable.

STAFF REQUIREMENTS:

 

Qualified mental health professional (QMHP) refers to a clinician in the human services field who is trained and experienced in providing psychiatric or mental health services to individuals who have a primary or secondary psychiatric diagnosis. In the Commonwealth of Virginia, authorized professionals and minimal qualifications for a QMHP are as follows:

 

  1. A doctor of medicine licensed in Virginia;

 

  1. A doctor of medicine or osteopathy, specializing in psychiatry and licensed in Virginia;

 

  1. An individual with at least a masters or doctoral degree in psychology from an accredited college or university with at least one year of clinical experience;

 

  1. Social Worker: An individual with a bachelor's degree in human services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling or other degree deemed equivalent to those described) from an accredited college or university with at least one year of clinical experience providing direct services to individuals with a diagnosis of mental illness;

 

  1. An individual with at least a bachelor's degree from an accredited college in an unrelated field that includes at least 15 semester credits (or equivalent credits based on a trimester system) in a human services field and who has at least three years clinical experience.

 

  1. A registered nurse licensed in Virginia with at least one year of clinical experience;

 

 

  1. A Registered Psychiatric Rehabilitation Provider (RPRP) registered with the United States Psychiatric Rehabilitation Association ( USPRA) as of January 1, 2001;

 

  1. Any other licensed mental health professional

 

  • Clinical experience means providing direct behavioral health services to individuals with mental illness, intellectual disability or receiving gerontology or special education services. The clinician must have documented experience in the specific field that they are working. They must have experience with implementing individual service plans. The clinical experience may include supervised internships, practicum's, and field experience.

 

  1. QMHP's will have all required company trainings met within a 15 day period after hire and a tuberculosis screening within a 30 day period after hire.

 

  1. Have no substantiated findings on the criminal history or central registry background checks.

 

  1. All facilities or services shall require that all applicants for employment disclose any criminal conviction. The impact of this information on a decision regarding employment shall be based upon the offense in relationship to the job for which the applicant is applying. Any barrier crime convictions, regardless of when it occurred, will cause an applicant to be ineligible for the position as defined by the Code of Virginia 63.2-1719 and 37.2-314.

KNOWLEDGE, SKILLS, AND ABILITIES REQURIED:

 

  1. Knowledge of adolescent/adult mental health/substance abuse disorders and consistent treatment approaches.

 

  1. Ability to implement, then convey therapeutic interventions, outcomes and consumer progress on progress notes.

 

  1. Obtain and maintain all certification/training requirements as required by the agency.

 

  1. Advanced skills in PC/Word processing/database/spreadsheets/systems, and integrating the advanced skills to produce complex reports and documents.

 

  1. Ability to apply therapeutic techniques. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.

 

  1. Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills.

 

  1. Ability to handle multiple tasks and meet critical deadlines. Ability to meet short and long-term goals and project deadlines.

 

  1. Respect for the confidentiality of all communications required within job duties, including client records and documents, and electronic transmission by voice, data, and wire.

 

 

Positions Supervised: QMHP-A staff may be required to supervise other QMHP staff.  Supervision of mental health services that are of a supportive or maintenance nature, such as mental health supports, shall be provided by a QMHP-A. 

 

Physical Demands: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.

 

Working Conditions: Work is performed in the community and/or office setting; Work may expose QMHP to contagious or infections diseases; Work may expose QMHP to dangerous and volatile situations; which could result in bodily injury.
