We are currently looking to bring on experienced Licensed Team Leader to serve as a member of our ACTT service.

DUTIES

1.Directs the day-to-day clinical operations of the ACT team

Manages staff work schedule to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours

Leads daily staff meetings and treatment planning meetings

Continuously evaluates the status of clients and plans/coordinates treatment activities to ensure immediate attention to their changing needs

2.Directs and/or coordinates for each client:

Admission and treatment

Rehabilitation and support services in coordination with the psychiatrist

Admission interview, initial assessment , and initial treatment plan

Staffing

Client-centered comprehensive assessment of psychiatric history and diagnosis

3.Initiates and maintains relationships with stakeholders including but not limited to (other staff, law enforcement, other humans service providers, and community resources)

4.Manages staff schedules, timesheets, documentation, performance/ clinical supervisions, trainings, and the development of new staff.

5.Manages the ACT program budget

6.Participates in Quality Assurance activities such as peer reviews and record reviews as needed

7.Performs on-call duty for crisis intervention and for staff supervision and consultation

8.Ensures client records are maintained to remain in compliance with agency policies.

9.Overseeing the administrative operations of the team;

10.Providing clinical oversight of services in conjunction with the Psychiatric Care Provider; as well as clinical supervision;

11.Supervising team members to assure the delivery of best and ethical practices;

12.Directly providing services to ACT service beneficiaries, where a therapeutic relationship is developed between ACT service beneficiaries and the Team Leader. Example roles include:

assuming an active role in screening referrals and assessing beneficiaries at

intake;

acting as a lead clinician, therefore working closely with a select group of service

beneficiaries who can benefit from the Team Leader's clinical expertise;

modeling behaviors through service provision for the purpose of clinical

supervision;

participating in person-centered planning meetings; and

working with beneficiary's natural supports.

10.The Team Leader is exclusively dedicated to the ACT team, with no responsibilities to other roles outside of the ACT team. Only one Qualified Professional shall assume the role as Team Leader. Qualifications are set forth in Table 2 and Division of Medical Assistance Clinical

QUALIFICATIONS

ACT Team Lead:

1.The Team Leader must be a licensed mental health professional holding any of the following licenses: Licensed Psychologist, Licensed Psychological Associate, Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, Licensed Psychiatric Nurse Practitioner, Clinical Nurse Specialist certified as an advanced practice psychiatric clinical nurse specialist.

2.An associate level licensed professional may serve as the team leader conditional upon being fully licensed within 30 months from the effective date of this policy. For associate level licensed team leaders hired after the effective date of this policy, the 30-month timeline begins at date of hire.

3.The team leader must have three years of post-graduate clinical experience with SMI/SPMI.
